Florida Department of Health Okaloosa County: Pediatric Dental Clinic

The Dental Clinic in Crestview provides services to children ages 4 to 18 and who are on Medicaid.
We accept Molina, Wellcare/Staywell, Sunshine, DentaQuest, Liberty Dental, Dental Health and Wellness and United Health Care.
The clinic also accepts Argus and DentaQuest Healthy Kids plans.
We recognize the need for children's dental services. The goal of our dental clinic is to provide oral health services to the underserved population in Okaloosa County. Our dental team consists of dentists, dental assistants, and other professionally trained staff.
